SCOTT TOWNSHIP BALL PARK
RELEASE OF LIABILITY
I, the undersigned, hereby declare that I am the parent and/or guardian of the above-named "PLAYER" and, as such, give any consent for said PLAYER to participate in any and all scheduled activities of the Scott Township Baseball and/or Softball Leagues, from tee-ball through U18 Slow Pitch and all divisions included in the leagues for this season. I further recognize the risks arising from said PLAYER's participation in said programs. In return for Scott Township Baseball and/or Softball Leagues allowing PLAYER to participate in said programs, I hereby release and hold harmless and agree to indemnify the Scott Township Baseball and/or Softball Leagues, Scott Township Trustee, Scott Township Board, Scott Township Recreation Booster Club Board, managers, coaches and any other persons connected with said programs, and the agents, heirs, assigns, successors, or estates from any and all claims for damages arising from or related to any and all injuries, physical or otherwise, sustained during PLAYER's participation in said programs.
I, the undersigned, hereby assume ALL of the risks of participating in any/all of the activities associated with Scott Township Ball Park, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that PLAYER is physically fit, has sufficiently prepared or trained for participation in any/all activities in said programs, and has not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude PLAYER’s participation in any/all activities in said programs.
I understand while participating in this activity, PLAYER may be photographed. I agree to allow PLAYER’s photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and directors.
The Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL AS GUARDIAN OF PLAYER.
